Therapeutic Management of Canine Immune-Mediated Hemolytic Anemia
Immune-mediated hemolytic anemia (IMHA) is a life-threatening disorder that requires timely therapeutic intervention to control immune-mediated erythrocyte destruction while maintaining patient stability. Once a diagnosis of IMHA has been established, treatment should be directed not only at suppressing the immune response but also at supporting the patient through the acute phase of disease. However, initiating therapy without adequately evaluating the underlying cause may be detrimental, particularly in cases of secondary IMHA. Glucocorticoids: The Foundation of Therapy
Glucocorticoids remain the cornerstone of treatment for canine IMHA. In many patients, remission can be achieved with glucocorticoid therapy alone1.
The recommended initial treatment for dogs is prednisolone at 2 mg/kg body weight orally once daily. When oral administration is not feasible, injectable dexamethasone at 0.2–0.4 mg/kg body weight intravenously once daily may be used as an alternative1.
The primary objective of glucocorticoid therapy is to suppress the immune response responsible for erythrocyte destruction, thereby reducing ongoing hemolysis and allowing recovery of circulating red blood cell mass.
When Additional Immunosuppression Is Needed
Although many dogs respond to glucocorticoids alone, some patients present with severe disease or fail to achieve adequate control with corticosteroid therapy.
In such situations, additional immunosuppressive agents may be incorporated into the treatment protocol2.

- Cyclosporine

According to the therapeutic recommendations described, transfusion may become lifesaving in dogs with:
- Packed cell volume (PCV) below 15%
- Severe tachycardia
- Tachypnea
- Marked lethargy
- Mental dullness1
Because IMHA is characterized by ongoing immune-mediated destruction of erythrocytes, transfusion introduces additional red blood cells into an environment where excessive RBC destruction is already occurring. Consequently, transfusion carries the potential risk of "fueling the fire," emphasizing the importance of careful patient selection1.
Selecting the Appropriate Blood Product
When transfusion is considered necessary, packed red blood cells (pRBCs) are preferred over whole blood because they provide erythrocytes without unnecessary plasma components2.
Whole blood remains an acceptable option during emergencies when packed red blood cells are unavailable2.
Following transfusion, the recommended target packed cell volume is approximately 25% or higher, providing sufficient oxygen-carrying capacity while avoiding unnecessary transfusion2.
Practical Clinical Insights
- Confirm the diagnosis carefully before initiating immunosuppressive therapy, particularly to exclude secondary IMHA or non-immune causes of hemolytic anemia.
- Glucocorticoids remain the first-line treatment and may achieve remission in many patients when instituted appropriately.
- Consider additional immunosuppressive agents in dogs with severe disease or inadequate response to corticosteroid therapy alone.
- Base transfusion decisions on the patient's overall clinical condition rather than hematocrit values alone.
- Packed red blood cells are generally preferred for transfusion, while whole blood may be reserved for emergency situations.
- Remember that transfusion provides supportive care and does not halt the underlying immune-mediated destruction of erythrocytes.
