Regenerative and Adjunctive Therapies for Canine Osteoarthritis

Managing canine osteoarthritis (OA) often requires more than conventional pain control. While foundational management and pharmacological therapies remain central to treatment, regenerative and adjunctive interventions can provide additional support for selected patients. These therapies aim to improve joint function, address inflammation, and complement an individualized multimodal management plan rather than replace established treatment strategies. Their role should be determined based on the patient's clinical stage, overall health status, and treatment objectives. 

For practicing veterinarians, understanding where these therapies fit within routine OA management can help optimize patient outcomes while setting realistic expectations for pet owners. 

Intra-Articular Therapies: Targeting the Affected Joint 

Intra-articular therapies offer a localized approach to managing osteoarthritis by acting directly within the affected joint. 

Hyaluronic acid is used to restore the viscosity of synovial fluid, thereby supporting joint lubrication and function. Improving the quality of synovial fluid may contribute to better joint movement and comfort as part of a broader treatment strategy. 

Corticosteroid injections can provide short-term relief during inflammatory flare-ups. However, repeated long-term administration is discouraged because of the potential risk of chondrotoxicity1,2

Patient selection and appropriate case evaluation remain important when considering intra-articular interventions, ensuring that treatment objectives align with the patient's clinical condition. 

Regenerative Medicine: Emerging Therapeutic Options 

Regenerative medicine has become an important area of interest in canine OA management. Two therapies highlighted within multimodal treatment are platelet-rich plasma (PRP) and mesenchymal stem cell (MSC) therapy. 

These approaches aim to modulate joint inflammation while promoting tissue repair within the affected joint. Rather than functioning as conventional analgesics, they seek to improve the joint environment by supporting biological repair processes. 

Although these therapies have shown encouraging clinical potential, current evidence indicates that further standardization and larger-scale clinical evaluation are needed to better establish their efficacy and long-term safety2

For clinicians, regenerative therapies should therefore be considered as adjunctive options within a carefully planned multimodal treatment programme. 

Nutraceuticals and Dietary Support 

Nutritional interventions can also complement the management of canine OA. Therapeutic diets enriched with omega-3 fatty acids, specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), have demonstrated strong anti-inflammatory effects and may contribute to improved patient comfort3,4,5

Other commonly used nutraceuticals have shown varying levels of support. Glucosamine and chondroitin sulfate continue to be widely used; however, their results remain inconclusive. Green-lipped mussel extract and undenatured type II collagen (UC-II) have demonstrated potential benefits in some patients and may be considered as adjunctive options within a comprehensive management plan. 

When discussing nutraceuticals with pet owners, it is important to position them as supportive therapies that complement, rather than replace, established medical and rehabilitation strategies. 

Integrating Adjunctive Therapies into Clinical Practice 

Successful OA management requires selecting therapies that match the individual patient's stage of disease and clinical needs. Adjunctive treatments should be integrated thoughtfully alongside weight management, rehabilitation, and pharmacological therapy as part of a coordinated multimodal approach. 

Factors such as treatment availability, owner compliance, and financial considerations may also influence clinical decision-making. Open communication regarding expected outcomes helps establish realistic goals while supporting long-term adherence to the treatment plan. 

Practical Clinical Insights 

  • Consider intra-articular hyaluronic acid to support synovial joint function where clinically appropriate.
  • Reserve corticosteroid injections for selected inflammatory flare-ups, avoiding repeated long-term use because of potential chondrotoxicity.
  • Use PRP and MSC therapies as adjunctive options within a multimodal treatment strategy rather than standalone interventions.
  • Recommend omega-3-enriched therapeutic diets as part of long-term OA management.
  • Discuss nutraceuticals with balanced expectations, recognizing that some products have stronger clinical support than others.

Conclusion 

Regenerative and adjunctive therapies can strengthen a multimodal approach to canine osteoarthritis by complementing established medical and rehabilitation strategies. Careful patient selection, individualized treatment planning, and realistic owner expectations remain essential for integrating these therapies into everyday clinical practice while supporting long-term mobility and quality of life (Hu and Li, 2019; Upchurch et al., 2022; Franklin and Cook, 2023).
